PLEASE READ CAREFULLY

By signing this document, you confirm that you have read, understood, and agreed to all studio policies.
You also acknowledge that you and your artist have agreed to a specific design and appointment date.
This agreement protects both you and the studio, ensuring that all tattoo procedures are performed safely and transparently.

GENERAL TERMS

• A deposit is required to secure an appointment date and the design agreed upon with the artist.
Deposits are non-refundable and will be applied to the final session once the tattoo is completed.
• By signing this form, both the client and the artist confirm agreement on the tattoo design and appointment date.• Clients must give at least 7 days notice for any rescheduling or design changes.• Design Change Policy (less than 7 days notice):
If a design change is requested within 7 days of the appointment, the deposit may be forfeited or the appointment may be rescheduled at the artist’s discretion.
• Payment is due at the end of each session.• Clients must arrive on time.
If delayed, please contact the studio immediately.
• Session time includes stencil placement, preparation, and consultation.• Clients must remain still and minimize distractions during the tattoo process.• No guests are allowed in the tattoo area.• No drugs or alcohol before or during the session.• Tattoos are hand-crafted artwork. Minor variations are natural and part of the artistic process.

LARGE OR MULTI-SESSION PROJECTS

• Large-scale tattoos may require multiple sessions.• Sessions are usually scheduled approximately once per month until completion.• Session length and schedule are determined by the artist.

AGE AND CONSENT

• Clients must be 19 years of age or older.• Government-issued photo ID may be requested for verification.

HEALTH & SAFETY ACKNOWLEDGMENT

• I confirm that I am not under the influence of drugs or alcohol.• I have disclosed any relevant medical conditions (including diabetes, skin disorders, allergies, or bloodborne illnesses).• I understand that the studio follows strict hygiene standards including:sterilized equipmentsingle-use needlesproper disposal of biohazard materials.• I release Lotus Bell Ink Atelier and the artist from liability for any complications caused by undisclosed medical conditions or failure to follow aftercare instructions.

AFTERCARE ACKNOWLEDGMENT

• I agree to follow all aftercare instructions provided by the artist.• Failure to follow these instructions may affect healing and final results.• The artist is not responsible for issues caused by improper aftercare.

PHOTO & VIDEO CONSENT

• I consent to photos or videos of my tattoo being taken.• These images may be used for the artist’s portfolio, website, or social media.

RESULTS & LIABILITY

• I understand that tattoo results may vary depending on skin type, placement, and healing process.• No specific outcome can be guaranteed.• I release Lotus Bell Ink Atelier and the artist from any legal claims related to the tattoo procedure.• I consent to emergency medical assistance being contacted if necessary.

COVER-UP DISCLAIMER (If Applicable)

• Cover-up tattoos or tattoos over scars may require multiple sessions.• Final results cannot be guaranteed.

FINAL ACKNOWLEDGMENT

By signing below, I confirm that:• I have read and understood all policies.
• I agree to the deposit and rescheduling policies.
• I voluntarily consent to receive a tattoo from Lotus Bell Ink Atelier.
